The pectoralis major reconstruction of sternal wounds is a great life saving procedure. Nevertheless, when being performed in neonates, the resulting functional deficits and the implications on future professional careers or sport activity choices have to be taken in consideration. The preservation of the neurovascular pedicle, utilizing the thoracoacromial vessels instead of the internal mam-mary supply, may keep the muscle in a more anatomical and functional position.
