A 34-year-old man was referred to us with gunshot wounds caused by a shotgun fired at an approximate distance of 100 m. His chest and abdomen were impregnated with shots, creating a star-burst appearance (Fig 1).
After abdominal repair, the existence of dyspnea prompted us to perform an echocardiographic scan, which revealed moderate pericardial effusion and hematoma around the aortic arch. A 64-slice computed tomographic scan showed a hollow caused by the shot in the anterior of the aortic arch; the shot itself was trapped in the posterior side of the arch, begetting the slight hematoma (Fig 2). The shot was surgically removed and the perforation was repaired (Fig 3).
