A 26-year-old woman was referred to our center from another hospital for pacemaker box replacement. She had undergone implantation of an epigastric permanent pacemaker 2 years previously because of complete heart block. At clinical examination the generator could not be detected. Using an imaging intensifier, the generator was located in the pouch of Douglas, between the rectum and uterus (Fig 1), and was retrieved using a mini-Pfannenstiel incision.
