Significant attention has been given to harnessing the effects of cardiopulmonary bypass on young patients. With constant advances in surgical techniques, the limitations to pediatric cardiac surgeons are not so much technical as they are physiologic. When tiny infants and neonates are exposed to cardiopulmonary bypass (CPB), they are exposed to biologic extremes in temperature, flow rates, and pressures, as well as to system extremes related to hemodilution, surface area exposure, and the effects of mechanical systems on blood. Overall, this can create a significant inflammatory response, which in infants can translate into disturbances in the coagulation system, neurologic outcomes and the overall hemodynamics seen following surgery. The "Symposium on Harnessing the Effects of Neonatal Cardiopulmonary Bypass," which took place on September 21, 2005, during The Fourth World Congress of Pediatric Cardiology and Cardiac Surgery in Buenos Aires, Argentina, was organized to bring together recognized experts in the field of neonatal cardiac care to address the current state of these relevant issues in infant CPB, as they relate to outcome.

We greatly appreciate the generous support from Bayer Pharmaceuticals Corporation through an unrestricted educational grant that made the educational program possible. In addition, the gracious hospitality and extraordinary efforts of the co-chairs (Drs. Horacio Capelli and Guillermo Kreutzer) and organizing committee (Drs. Horacio Faella, Christian Kreutzer, Pablo Marantz, and Andres Schlichter) of The Fourth World Congress should be commended. This was a "standing-room" only session and they ensured that the audience was comfortable and that the room provided the appropriate setting for a lively and engaging meeting. Finally, we are pleased to be able to record the proceedings of this memorable symposium in this Supplement to The Annals of Thoracic Surgery.
