We enjoyed reading the description of the series of triangular anterior leaflet plications performed by Seitelberger and colleagues [1]. We prefer to employ this technique when the anterior segment requiring adjustment is too small to require a formal anterior leaflet resection, such as when there is a minor segmental prolapse of the anterior leaflet. However, we feel obliged to note that this is not an entirely new procedure. In 1992 McIntosh and colleagues [2] published a series of anterior leaflet plications performed via the aortic root in association with septal myotomy and myectomy. Likewise Alvarez and colleagues [3] described plication of a flail anterior leaflet in 1993.
