To the Editor:

We would like to thank Dr Robicsek [1] for reminding us that there is nothing new under the sun. We did not find this original article by Dr Bailey when we were reviewing the literature. Although our technique is different, the end result is the same, providing a pop-off for the right ventricle in cases of severe pulmonary hypertension. Robicsek's opinion that it is "a welcome event to see the rebirth of this technique" could not be more prophetic. Since the publication of the article [2] we have added another four sites to our database: two in Mexico (ie, Mexico City and Monterrey) and two in India (ie, Mumbai and Bangalore). Thomas Friedman recently published a new book, The World is Flat, and we are seeing just how flat it is becoming. We are now being contacted by cardiac surgeons around the world from the Internet who have accessed this article through CTSNET and who want to perform or have performed this procedure. Every day as technology improves we are able to communicate our discoveries (or rediscoveries as the case may be) to our colleagues instantly and improve patient care. Certainly this aspect of the flattening of the world is an example of the good that can come from globalization.

We want to thank Dr Robicsek for reminding us that we should more carefully review the history of our discoveries before we publish them.
