I enjoyed the article on an alternative technique for orthotopic cardiac transplantation by Dr Tsilimingas [1] in The Annals. I am pleased that his approach to cardiac transplantation is similar to that of my group, but I regret that a previous report by my colleagues and I [2] escaped his references. Our article had almost the same title as his and almost the same technique to prevent tension, torsion, and kinking of the anastomosis, particularly in the case of small donor hearts. We left the posterior aspect of the right atrial wall as a bridge to connect the superior and inferior venae cava. The only difference is that Dr Tsilimingas added telescopic anastomosis of the superior vena cava. I hope Dr Tsilimingas will enjoy reading our article.
