We appreciate Sunar and Durans interest in our article and the opportunity to address their comments. Regarding valve preservation by simple excision "vegetectomy" for brucella endocarditis, the surgeon must realize that brucella is a slowly destructive organism, that can cause tissue ulceration, abscess, and large vegetations, and may be difficult to eradicate from cardiac valves with palliative surgery alone. In our clinical experience, vegetations occur after destruction of the valvular endothelial layer and anatomic integrity; consequently, vegetectomy would not be a sufficient operation. Radical surgery combined with antibiotic therapy should be used in the patients with complicated valvular disease in brucella endocarditis.
