Intramyocardial echinococcus cyst

A 40-year-old man from the Northern Territories of Pakistan was admitted to the Aga Khan University Hospital because of cough and chest heaviness. A chest radiograph (A) revealed a large mass on the left heart border. A computed axial tomographic (CAT) scan (B) revealed the cystic structure of a mass 7 x 8 cm. At thoracotomy, the normal-appearing pericardial sac was entered and an intramyocardial mass was identified beneath the left anterior descending coronary artery (C). The cyst was aspirated. An incision was made through the epicardium and the echinococcus endocyst was removed (D). The cavity was lavaged with Albendazole and left open. The patient was treated with Albendazole and had an uneventful recovery.
