Ischemia-reperfusion injury in cardiac surgery

Edited by Friedhelm Beyersdorf, MD, 2000, Georgetown, TX, Landes Bioscience, 227 pp, 51 figures, 20 tables, $119.00, ISBN: 1-58706-002-7

Irving L. Kron, MDa

a Charlottesville, Virginia, USA

This is a complete reference on ischemia-reperfusion injury as it applies to the science and practice of cardiothoracic surgery. This information is of significant value to the researcher, clinical surgeon, and students and residents who are interested in myocardial protection. The book is an excellent one and it is quite complete. The editor, Dr Beyersdorf, is an authority on ischemia reperfusion injury and he has selected an excellent group of contributors for this.

There are 19 chapters broken into four sections. These sections include the basic science of ischemia reperfusion injury, methods of prevention, intravenous metabolic support and surgical techniques. These chapters have been prepared by a total of 36 contributors.

Overall, the book is quite to easy to read. The organization is excellent and takes the reader from the basic science through clinical therapies that relate to ischemia reperfusion injury. The first chapter entitled, "Harnessing the Cardioprotective Potential of Nitric Oxide in Non-surgical and Surgical Ischemic-Reperfusion Injury," by Vinten-Johansen and Ronson is a thorough analysis of this entity. This chapter is must reading for anyone interested in getting into experimental issues that relate to this topic. Following thorough review of the basic science of metabolic support of the heart, there is an excellent section on clinical applications that relate to various techniques of myocardial protection. This discusses in detail most strategies that are presently used. The section by Donatelli and colleagues on acute myocardial infarction and cardiogenic shock is particularly valuable.

This is a very good text that clearly lives up to its introduction. The only thing missing is an overall clinical view on off-pump strategies. I believe this text can be recommended as a valuable addition to the library of anyone interested in myocardial protection.
