To the Editor
I thank Dr Gerdes and Dr Sievers for their gracious and thoughtful letter in support of my hypothesis that end-hole aortic cannulas direct flow at the left carotid orifice, thus explaining the preponderance of left-sided hemispheric strokes in coronary artery bypass grafting [1]. The references they cite offer additional confirmation of this hypothesis.
I am in complete agreement with them that the end hole must be closed so that all flow is directed through the side holes. I was unaware of the existence of cannulas with both end holes and side holes, and I thank the authors for clarifying this point. I am hopeful that my article will be a stimulus for further research in this area.
