We thank Drs Konstantinov and Hermansson for their interest in our report. We think that there is no substantial difference between their opinion and the conclusions of our report. According to us, video-assisted thoracoscopy may be a very useful procedure in selected cases. We stated that in emergency situations or in the presence of a cardiac or an intra-pericardial vascular injury, sternotomy is appropriate. On the other hand, we agree with Drs Konstatinov and Hermansson about the lack of indication to open thoracotomy when video-assisted thoracoscopy is unsuccessful, provided that thoracoscopy was carried out by an experienced thoracic surgeon. We would like to point out that patient no. 2 of our paper was referred to one of us following unsuccessful video-assisted thoracoscopy and right thoracotomy that was performed in another hospital.
