We appreciate Dr Chandras kind comments and are in agreement with his points. An appropriately functional system, an attitude expectant of quality, team relationships, and surgeon involvement are all vitally important in achieving good surgical results. This is a difficult time in health care, and surgeon involvement is perhaps more important than ever. As patients become cases and surgery becomes a business, we are enticed to turn toward a business ethic and abandon our traditional values. In addition, many historical patient advocates have fallen by the wayside. We need to reemphasize our role as caregivers and remain strong in our position of patient advocate, for we will define what is the lower limit of cost-sensitive quality care.
