The Ad Hoc Committee on Circulatory Support and Thoracic Transplantation of The Society of Thoracic Surgeons takes great pride in the publication of these manuscripts presented at the Fifth STS-sponsored International Conference on Circulatory Support Devices for Severe Cardiac Failure in New York, NY, September 1517, 2000. These symposia have occurred every 3 years since 1988. Circulatory Support 2000 was graciously hosted by the Department of Surgery at Columbia University who presented an outstanding site visit program featuring their extensive clinical and research efforts in circulatory support. The formal program of the symposium featured an opening session on acute cardiac failure with applications of devices for postcardiotomy, postmyocardial infarction and myocarditis-induced cardiogenic shock. Subsequent sessions featured bridge to transplantation and an excellent review of myocardial recovery after circulatory support. A stimulating session focused on newly developed or developing devices, and a "soap-box symposium" featured future devices. Important consideration was given to the economics of devices as well as the establishment of a worldwide database. In keeping with the original intent of these meetings, there was strong emphasis on the practical aspects of clinical applications by way of case presentations and audience participation. The success of this symposium was evidenced by the multiple requests that it be held every 1 to 2 years rather than every 3 years. Finally, the committee is deeply indebted to the efforts of the Guest Editors, Drs George J. Magovern, Sr, Yoshifumi Naka, Mehmet C. Oz, and D. Glenn Pennington, and the tireless efforts of Dr L. Henry Edmunds, Carol L. Blasberg and staff to ensure the earliest publication of the proceedings of these symposia since their inception. We believe the content and the timeliness of the publication will make this supplement a staple for all students of mechanical circulatory support for cardiac failure.
