Congenital Heart Surgery Nomenclature and Database Project: diseases of the pericardium

Abstract

The extant nomenclature for pericardial disease is reviewed for the purpose of establishing a unified reporting system. The subject was debated and reviewed by members of the STS-Congenital Heart Surgery Database Committee and representatives from the European Association for Cardiothoracic Surgery. All efforts were made to include all relevant nomenclature categories, using synonyms where appropriate. No classification system has been reported. The groups were based on disease processes, and include: effusive pericarditis (pericardial effusion), constrictive pericarditis, cardiac tamponade, postoperative pericardial effusion, postoperative cardiac tamponade, postpericardiotomy syndrome, congenital defect, neoplastic process, benign mass, pericardial cyst, pneumopericardium, and chylopericardium. A comprehensive database set is presented that is based on a hierarchical scheme. Data are entered at various levels of complexity and detail, which can be determined by the clinician. These data can lay the foundation for comprehensive risk stratification analyses. A minimum database set is also presented that will allow for data sharing and would lend itself to basic interpretation of trends.

I. Background

A complete discussion of diseases of the pericardium includes a spectrum of acquired and congenital problems [15]. Structural deficiencies, infectious and inflammatory processes, and neoplastic lesions must all be considered. Though a classification scheme for the range of pericardial disease has not been described, this document makes every attempt to include the diverse group of problems encountered when treating patients with pericardial disease.

Pericardial disease: A structural or functional abnormality of the visceral or parietal pericardium that may, or may not, have a significant impact on cardiac function.

Pericardial disease, NOS: Pericardial disease not otherwise specified.
Pericardial disease, Effusive pericarditis (pericardial effusion): Inflammatory stimulation of the pericardium that results in the accumulation of appreciable amounts of pericardial fluid.
Pericardial disease, Constrictive pericarditis: An inflammatory process of the fibrous and serous layers of the pericardium that leads to pericardial thickening and compression of the cardiac chambers, ultimately with an associated significant reduction in cardiac function.
Pericardial disease, Cardiac tamponade: Cardiac compression.
Pericardial disease, Postoperative pericardial effusion: A fluid collection in the pericardium, after a surgical intervention, that requires treatment
Pericardial disease, Postoperative cardiac tamponade: Cardiac compression after surgical intervention.
Pericardial disease, Postpericardiotomy syndrome: Pericardial inflammation secondary to either cardiac muscle damage or surgical cardiotomy, associated with pain, friction rub, and electrocardiographic changes suggestive of ischemia.
Pericardial disease, Congenital defect: Congenital defects of the pericardium comprise a rare group of lesions that range from isolated defects to complete absence of the pericardium. These lesions may be completely asymptomatic, but often are associated with marked hemodynamic compromise.
Pericardial disease, Neoplastic processes: Malignant disease involving pericardium or pericardial space.
Pericardial disease, Benign mass: A benign mass involving pericardium or pericardial space.
Pericardial disease, Pericardial cyst: Cystic lesion found in close proximity, though rarely communicating with, the pericardium, commonly containing clear yellow fluid, and rarely associated with symptoms.
Pericardial disease, Pneumopericardium: Finding of air within the pericardium.
Pericardial disease, Chylopericardium: Chylous effusive process of the pericardium.
