Inflammatory pseudotumors of the lung

Cerfolio and colleagues [1] report of nearly 50 years experience of 23 pseudotumors of the lung is a model of a well-conducted retrospective analysis of a rare pathologic entity. The suggestion, however, that there may exist two distinct clinical/pathologic types of pseudotumor as attempted in their Table 2, must be questioned. The data may simply reflect length of time from inception to diagnosis.

In an earlier report [2] taken from 10 years experience with a similar population there were two definitely invasive cases (largest diameters, 7 and 10 cm), two cases with adhesions to local nonpulmonary tissue (3.3 and 4 cm), and two cases without invasion or adhesions (1.5 and 1.9 cm), ie, the pathologic changes were stratified by size. As a consequence considerable doubt about the suggestion of two distinct types of pseudotumor based on invasion of surrounding tissue must remain, especially in the absence of data demonstrating tumor size.
