We were interested in the article named "One-Stage Surgical Procedure for Bilateral Lung and Liver Hydatid Cysts" by Dhaliwal and Kalkat that was published in your journal [1]. Echinococcosis is also an important problem in our country. It is well known that the treatment methods applied to patients with multiple involvements decrease the mortality and morbidity rates and the cost of treatment and make the hospitalization duration short. For this reason, it may appear that a single operation instead of three individual operations, which was introduced in the article, is a more convenient approach. In spite of this, the number of cases is insufficient to base a decision on. In addition, percutaneous needle aspiration together with ultrasonography has recently been performed successfully in liver hydatid cysts, which are unilocalized and without complications [2, 3]. In our clinics, for patients with involvement of lungs and liver, we now apply primarily surgical treatment to both lungs with median sternotomy. After the operation, within 1 month the patients apply to the general surgery clinics and their liver hydatid cysts are treated with percutaneous needle aspiration. This process has been applied in 9 cases between 1994 and 1997, and we saw no mortality, morbidity, or recurrence.
We conclude that this approach is more convenient, and the process mentioned in the article may be convenient only for selected cases.
