Doctor Konertz suggested another technique to expose the mitral valve through the dome of the left atrium with the Cooley atrial retractor. Our own experience with minimally invasive aortic and mitral valve surgery dates back to February 1996. Since then, we have tried to improve the exposure of the mitral valve, including using the Cooley atrial retractor that Dr Konertz suggests. Our skin opening is approximately 6 x 6 cm, and any instruments placed inside this limited opening can obstruct the view of the mitral valve. Therefore, we elected to use traction stitches with pledgets as described [1], and this approach so far has given us better exposure of the mitral valve.
