A Proper Name for the Internal Mammary Artery?

Tsung O. Cheng, MDa

a Division of Cardiology, Department of Medicine, George Washington University Medical Center, 2150 Pennsylvania Ave, NW, Washington, DC 20037, USA

To the Editor

Kakadellis [1] originally wrote a letter to the Editor in 1996 concerning a proper name for the internal mammary artery. Then Salerno [2] wrote another letter to the Editor to comment on Kakadellis’s letter. In his reply, Kakadellis [3] suggested that Arthur Vineberg’s name be given to coronary artery bypass grafting. I beg to disagree with Kakadellis on this point.

Vineberg of McGill University in Montreal developed the technique of drawing the distally ligated internal mammary artery with its branches open and bleeding into a bluntly created tunnel in the outer or middle third of the left ventricular myocardium. By 1958 he had performed 1,500 animal experiments, with clinical implants beginning in 1950 [4]. His name consequently has always been identified with the internal mammary artery implant operation.

Rene Favaloro, who in 1968 performed the interposition of saphenous vein grafts for excised segments of diseased coronary arteries, reported in 1969 the use of saphenous vein for aortocoronary bypass grafting [5]. Although Favaloro was generally credited as the one who did the first successful human coronary artery bypass grafting operation, there was an earlier case of coronary artery bypass grafting performed by DeBakey and associates in 1964 but not reported until 1973 by Garrett, Dennis, and DeBakey [6]. George Green and associates [7] in 1968 reported the first substantial experience with the internal mammary artery as a bypass conduit and demonstrated excellent clinical results in a large number of patients in whom the operating microscope was used.
