I read with interest the article entitled "Traumatic SubarachnoidPleural Fistula" by Sarwal and associates in the December 1996 issue of The Annals [1]. Sarwal and associates presented 2 patients with traumatic subarachnoidpleural fistula and found 21 others reported in the literature.
I am writing to add 1 more patient with traumatic subarachnoidpleural fistula reported by me and my associates [2]. The patient was a 21-year-old man who had a gunshot injury of the thorax and the thoracic vertebral column resulting in paraplegia and a traumatic subarachnoidpleural fistula at the T3-T4 level. The patient underwent operation on the 35th day after the injury, at which time the fistula was closed successfully with a fat pad and pieces of muscle covered by a pleural flap.
