We read with interest the article by Roux and associates describing their new technique and instrument for sternal osteosynthesis [1].
We are using a similar technique for sternal osteosynthesis using any standard sternal retractor. After the required number of wires are passed through both halves of the sternum and crossed over, the second and the penultimate wire are joined together by twisting their ends. Then any sternal retractor is put between these wire loops and is opened gradually until adequate apposition and impaction of sternal halves are obtained. The blades of the retractor engage in the wire loops and perform the function of pulleys. Once adequate apposition, alignment, and impaction are obtained, the remaining wires are tightened, followed by the looped wires. The procedure can be carried out single-handedly.
Our method is similar to the one described by Roux and associates, but instead of using a specialized retractor, one can use any standard sternal retractor with the blades functioning as pulleys and obtain a similar result without incurring any additional expenditure on procuring a new retractor with pulleys.
