Invited commentary

Whether harvested utilizing a pedicle technique or a skeletonization technique, the internal thoracic arteries (ITAs) have been proven to be excellent conduits for myocardial revascularization procedures. Since the long-term failure rate for vein grafts is relatively high, some surgical groups have adopted an initial revascularization approach utilizing various combinations of ITAs, radial arteries, and gastroepiploic arteries (GEAs). As reported in this paper by Gurevitch and associates, a combination of both internal thoracic arteries and an occasional GEA were used to provide complete revascularization in 404 of 472 . . .
