DR LEONARDO LIMA (Paris, France): I congratulate Dr Burdon and his colleagues for the great work that they have done. I think that it is the beginning of a new era in cardiac surgery.
At La Pitie Hospital in Paris we did some experimental studies in pigs to assess the feasibility of LIMA-LAD grafting with or without a thoracoscope, before starting our clinical study. We think that to simplify that technique, for the LIMA-LAD anastomosis, the better approach is to harvest the LIMA thoracoscopically, and then to perform the anastomosis by means of a small left anterior thoracotomy (4 cm). We do believe that it is better and cheaper, in these cases, to carry out bypass grafting on a beating heart and without cardiopulmonary bypass. Myocardial protection, when it was necessary, was achieved by means of an intracoronary shunt.
Finally, I congratulate you again for this beautiful work. I believe that in the near future we will be able to bypass, thoracoscopically, more than one coronary artery.
