The authors of this article have demonstrated that double internal mammary artery grafting with the right artery passing through the transverse sinus is a feasible approach, but one has to question whether in general it is advisable. I have misgivings regarding the prudence of increasing the complexity of the coronary operation when many more simple and proven alternatives already exist. As someone who is not complacent and, I believe, in the majority, and who tends to regard coronary artery operations as palliative rather than curative, and who performs coronary artery operations with a low mortality routinely using the left internal mammary artery to the left anterior descending artery and vein grafts to other vessels requiring grafting, and who achieves good results, and
