Luciani and colleagues herein describe the course of two neonates with the rare combination of transposition of the great arteries/intact ventricular septum and severe, reactive pulmonary hypertension. This anatomic and physiologic combination is uniformly fatal without prompt recognition of the abnormal physiology and institution of intensive support geared at minimizing pulmonary vascular resistance and increasing intercirculatory mixing.
