DR DONALD B. DOTY (Salt Lake City, UT): Mr Westaby and associates have presented a series of 80 patients in whom the aortic valve was replaced using a porcine aortic root that is glutaraldehyde-fixed. This bioprosthesis is processed by Medtronic Company and is called the Freestyle valve because of the various methods in which the patient's own aortic root structures are used to support the valve rather than a prosthetic stent. A larger porcine valve is thereby accommodated in the aortic outflow tract so that flow properties are better than with stented valves.
Other unique features of the bioprosthesis include distention of the aortic sinuses during glutaraldehyde fixation while maintaining zero pressure on the valve leaflets. Alpha-aminooleic acid is added to reduce calcium deposition in the transplanted tissues. A thin Dacron covering is provided for an easy proximal suture line and to prevent shrinkage of the portion of the valve that is supported by the septal myocardium.
The results showed an acceptable 5% operative risk of death in patients mostly more than 70 years old and late deaths in another 8%, none valve-related. Valve competence
