To the Editor:
We read with interest the article by Forshaw and colleagues [1] concerning the efficacy of cardiopulmonary exercise testing before esophagectomy. The low perioperative mortality rate of 1.3% in such a major surgical procedure is to be commended and demonstrates the difficulties in using mortality as the only outcome endpoint in a small study group.
As discussed, we would support the likely cause for the lack of association between complications and cardiopulmonary exercise
