Medical Treatment for Acute Type A Aortic Dissection?

To the Editor:

We are writing with regard to the publication by Rampoldi and colleagues [1], which we read with great interest. However we believe that some aspects of this important article require further comments.

Of the 834 patients with acute type A aortic dissection, 148 patients (17.7%) were treated medically due to advanced age, severe comorbid illness, intramural hematoma, or refusal of intervention. In our opinion, advanced age is not generally a reason per se to refuse surgery.
