A 14-year-old white boy presented with syncope and a left carotid bruit. Arch aortogram showed narrowing of the distal aortic arch, left carotid stenosis, and small left subdavian and vertebral arteries. When aortic arch replacement was carried out a vertical septum was found in the distal arch. Histologic examination suggested this was ductai in origin.
