The left thoracotomy approach represents a safe alternative to median sternotomy for coronary artery bypass grafting in patients with pericardial adhesions. It provides an excellent exposure to the lateral aspect of the heart, and isolated circumflex coronary revascularization can advantogeously be performed via this approach in select cases.
