A 62-year-old man with no history of preexisting heart disease was seen in
cardiogenic shock. Prompt cardiac catheterization and aortography revealed
pericardial tamponade and aneurysms of the right and left sinuses of
Valsalva. Immediate sternotomy relieved the tamponade, which was secondary
to an aneurysm of the left sinus of Valsalva rupturing into the transverse
pericardial sinus. Endoaneurysmorrhaphy was performed successfully.
