Repair of Anteroseptal Ventricular Aneurysm

Anterior ventricular aneurysms are usually accompanied by fibrosis and dyskinetic tissue in the ventricular septum as well as in the anterior ventricular wall. Improved results have been obtained by including the septum in the repair of the aneurysm. Vein bypass grafts are directed to the circumflex and right coronary arteries with no attempt to revascularize the obliterated anterior descending coronary artery. Postoperative ventriculograms and arteriograms were obtained in 20 of the 29 patients. There have been 3 postoperative deaths. The majority of patients were clinically improved with evidence of improved ventricular function.
