The purpose of this communication is to report 5 patients in whom, following pulmonary embolectomy for massive thromboembolism, extensive pulmonary hemorrhagic infarction occurred; 4 patients died. Four of the patients had been operated upon using cardiopulmonary bypass, and 1 patient had a Trendelenburg embolectomy without the use of extracorporeal circulation.
The cause of this alarming condition is unclear, but because of the frequency with which we have encountered the complication, we have become increasingly more discriminative in the selection of patients for pulmonary embolectomy.
